Mandated Screenings And Immunizations

All children entering and attending school must be adequately immunized against Polio, Diphtheria, Measles, Mumps, Rubella, Hepatitis B, & Varicella(Chickenpox).

Without proper proof of immunization, children will not be permitted to attend school.

New York State Public Health Law also requires all children entering the Pre-K or Kindergarten and seventh grade be immunized against Hepatitis B. If you have any questions, please contact the appropriate school nurse.

Scoliosis- screening* every year for grades 6th, 7th, 8th

7th Graders- must have a physical examination* completed by end of seventh grade including: vision, hearing, height and weight. Exams dated 9/8/2008 or later are accepted.

6th Graders- are required to have two immunizations upon entering 6th grade.

  • Tdap booster (Tetanus, diptheria and pertussis)
  • Varicella vaccine (Chicken pox)- must have one of the following:
    • Date of Immunization
    • Positive titer showing immunity (blood work)
    • Doctors note verifying proof of disease

*You may choose to have your own Health Care Provider perform the physical or screening or set up one with the school's Nurse Practitioner. If completed outside of school please send in a copy for the file.
