School-Based Health Center (SBHC)
Adirondack Central School District's School-Based Health Center (Rome Health)
Rome Health’s Boonville Health Center, in collaboration with the Adirondack Central School District, has established a School-Based Health Center (SBHC) in Boonville. We are located at the Adirondack Middle/ High School, The school district will provide transportation from the elementary schools to our location for students enrolled in the program. A SBHC provides primary care and preventative health care services in school to those students enrolled in the program. These services include:
- Complete physical examinations
- Vision, hearing, and scoliosis screenings
- Immunizations, Flu vaccines
- Diagnosis and treatment of both short-term and long-term illnesses
- Health education and wellness promotion
- Mental health screenings and referrals
All students are eligible to enroll in the School-Based Health Center. We do not replace the student’s primary care doctor; we work with them. There are no out of pocket expenses for any service provided by the SBHC. The SBHC will bill the students’ insurance. If your child does not have health insurance coverage, or youre health insurance does not cover SBHC visits, there will be NO CHARGE. If you do not have health insurance coverage for your child and are interested in how you can receive information on free or low-cost insurance, please call the SBHC or contact Rome Health at 315.942.3500.
If you wish to enroll your child in a SBHC, please complete the SBHC enrollment forms, and return them to the School-Based Health Center as soon as possible.

Hi everyone! I’m so excited to be here as a provider for the School-Based Health Center. I’ve been a nurse for over 20 years, and I currently provide care for patients of all ages at the Boonville Family Care Office through Rome Health. Some of you may remember me from my time working at Adirondack Central School—I loved getting to know the kids and families, and I’m really looking forward to reconnecting. I am also a proud graduate of Adirondack and look forward to returning to the district.
I live right here in Boonville with my husband and our two sons, and we’re all very involved in the community. It means a lot to me to bring the School-Based Health Center back into the schools, and I’m excited to be a familiar, friendly face for students who need care during the school day.
I am so excited to join Leah as a provider in the School-Based Health Center! Nursing has been my passion for the past 13 years, and for the last 8 years I’ve had the privilege of serving our community as a nurse practitioner right here in Boonville. Now, I’m thrilled to expand my care to students and families through the SBHC.
Boonville has always been home for me. I graduated from Adirondack in 2009, and it feels wonderful to return not just as an alumna, but as a parent, too—both of my boys are proud Adirondack students. Being back in the same halls where I grew up is both exciting and deeply meaningful.
Beyond my work in healthcare, I’ve been a proud member of our local fire department since I was 14. Giving back and caring for this community has always been at the heart of who I am. Whether in the clinic or out in the community, my goal is to provide compassionate, accessible care that helps students thrive.
I look forward to meeting our students, families, and staff, and to being a resource for health, wellness, and support here at Adirondack!
